Information for Health Care Professionals

REFERRALS

This service requires a referral from a health care provider.

​

Patient eligibility for this service are as follows:

​

  1. Age 18-25 years

  2. Presumed myofascial dysfunction as cause of chronic pain

  3. Other remedial cause for chronic pain has been excluded

  4. No needle aversion

  5. Not currently pregnant

  6. Not immunocompromised


Referrals should include the following:

  • Send a referral with the subject line indicating “REFERRAL”

  • Reason for referral

  • Recent consult notes

  • Current medications and past medical history

  • Previous pain management modalities utilized

  • Patient contact information with email address and phone number

 

Referrals can be faxed or emailed to our clinic.
